Key Responsibilities
Careline Hospice Clinical Coordinators are responsible for
answering inquiries, disseminating information, and assisting with
the daily operations of the organization. The Clinical Coordinator
establishes and maintains effective, efficient systems of
communication flow between Clinical Supervisors, the clinical team,
patients, and their families.
In this role you will be responsible for:
Answering all phone calls and communicating with appropriate staff
for patient evaluation
Scheduling patients for visits
Processing all workflow daily
Preparing dashboards
Performing field staff bag check
Maintaining organized, effective, and efficient systems and
communication to ensure the continuity of quality patient care
delivered
Accurately data processing all necessary information into the EMR
software for immediate use
Maintaining documentation in EMR
Assisting supervisor with meetings
Required Qualifications
High School Diploma or GED
Interpersonal skills and ability to communicate professionally and
effectively with team members, patients, and caregivers
Knowledge of medical terminology
Demonstrate problem-solving and conflict-resolution skills,
organizational skills, and attention to detail
Strong verbal and written communication skills
Ability to operate computer equipment, FAX, and photocopy machines
necessary
Proficiency in Microsoft Office Suite, including Outlook, Word,
Excel, and PowerPoint
Desired Qualifications
Associate Degree preferred
1-year experience in a health care setting preferred
Familiarity with Electronic Medical Record (EMR) systems

Hours
Office Hours: 8:00 am - 5:00 pm, Monday through Friday
